For 8 years, EOM Home care has been a trusted provider of home care services in New York. We were founded on the belief that everyone deserves access to quality, compassionate care, regardless of their challenges. Over the years, we have developed expertise in supporting individuals facing mental health challenges and those with a history of homelessness, recognizing the unique needs and sensitivities involved.

EOM Homecare is pleased to offer Cluster Care Services. Cluster Care provides dedicated assistance for one to two hours, seven days a week, ensuring a minimum of 7 hours of support weekly. This model has proven highly successful in providing consistent support within the client’s least restrictive environment, fostering independence and long-term well-being. We understand that consistency and reliability are crucial, and Cluster Care is designed to deliver just that.

With 15 years of dedicated experience in home care, Izmo has worked tirelessly to advocate for clients, particularly those navigating mental health challenges. Izmo’s commitment to providing compassionate, client-centered support has been a cornerstone of EOM Home Cares approach, ensuring that individuals receive the care, respect, and resources they need to thrive in their own homes. Throughout his career, he has collaborated closely with healthcare teams, families, and community services, always striving to create a safe, empowering environment where clients can feel heard, understood, and valued.

Paloma Gefen

With 10 years of experience in home care, coupled with Palomas background as a special education teacher, she brings a unique and compassionate approach to supporting individuals facing homelessness and mental health challenges. Her work has been centered on understanding and addressing the complex needs of each client, advocating for their well-being, and providing tailored care that promotes dignity and independence. Having a deep understanding of the intersection between mental health support and home care needs , Paloma is committed to creating a safe, supportive environment where clients receive the care and resources they need to overcome barriers and achieve stability.
